The different adhesive restorative procedures (direct and indirect) might be associated with surgical procedures in case of treatment of highly damaged teeth with lesions (caries, fractures, external resorptions, periodontal diseases) involving the subgingival area and the supra-crestal periodontal attachment. The focus will be on the indications, the published classification (EJED 2010) and the related differentiated therapeutic options: coronal relocation of the margin, surgical exposure of the margins and clinical crown lengthening. The advantages and potentialities of the combined surgical-restorative procedures will be highlighted also in case of subgingival fractures, external root resorptions, periodontal diseases. Some "out-of-protocol” cases, not based on scientific evidence, will be presented, in order to show an even more conservative approach, to widen our clinical horizons.
