Syllabus

Course Objectives

The course is aimed at the analysis of the care aspects related to the person in conditions of vital instability. Course objectives: The student at the end of the course will have acquired the necessary knowledge to ascertain, plan, implement and evaluate the care and educational interventions, useful for taking care of the critical patient, using guidelines, protocols or good practices supported by evidence and scientific proof.
The student will be able to draw up nursing care plans from the knowledge and topics covered in the course lectures and will be able to recognise and prevent emergency situations and complications that may occur in different care settings.

Course Prerequisites

The knowledge required and recommended at entry refers to the contents of the disciplines characterizing the 1st and 2nd year. Basic knowledge of disciplines relating to anatomy and physiology, general and clinical pathology, microbiology and immunology are required.
The student must also have attended the preparatory courses in Clinical Medical-Surgical Nursing and Clinical Medical-Surgical Nursing.

Teaching Methods

lectures, clinical cases, laboratory experiences

Assessment Methods

The evaluation is carried out in the manner described below:
- a written test in the form of a test, with 30 questions, with multiple choice questions, of which only one answer is correct. Each correct question corresponds to obtaining 1 point, for a total of 30 points.
Students with DSA certified and recognized by the Unipv SAISD Center will be able to take advantage of the compensatory measures recognized to them

Contents

Background
Definition and fundamental components of the critical area
Definition of vital criticality
Skills characterizing the critical care nurse
Organizational and structural characteristics in critical areas
Factors determining the complexity of care
Humanization of care
Quality standards, structural, process and result indicators
Systematic approach to the critically ill patient according to the ABCDE model
GENERAL PRINCIPLES OF CARE IN CRITICAL AREA
changes related to ICU admission: physical, mental and emotional symptoms that persist after discharge from the ICU. He will acquire knowledge regarding preventive and rehabilitative interventions in the delirium ICU, pain management, sedation, early mobilization and rest and sleep.

Pain management: assessment and interventions
Management of delirium and neuromuscular syndrome
Pain-agitation-delirium triad
Pain, delirium and sedation rating scale
Sleep management: sources of disturbance and interventions
Early, progressive mobilisation, immobility

Clinical case discussion: the critically ill patient with haemodynamic and cardiological problems:
Starting from a clinical case, the student will acquire the knowledge to manage and educate the patient with rhythm alterations or heart failure/shock.
Clinical monitoring and evaluation (signs and symptoms)
Pain management: assessment and interventions
Life-threatening heart rhythms: prompt recognition and treatment
Arterial catheter management and invasive blood pressure monitoring
Central venous catheter management, central venous pressure monitoring, safe administration of drug therapy
Notes on organ replacements: ultrafiltration, dialysis, ECMO notes (Extracorporeal membrane oxygenation)
The emergency trolley
Induced therapeutic hypothermia
OUT-OF-HOSPITAL AID, THE NETWORKS AND THE PRINCIPLES OF EVALUATION OF PERSONS WITH DYSPNEA (Cosi)

The student, starting from a clinical case (person with main symptom: dyspnoea), will be able to activate the rescue chain, then applicable in its principles to any working environment in which he will find himself operating. The student will be able to analyze and understand the clinical picture presented, will understand how to activate rescue services, safety problems at the scene, carry out an assessment of the person in a critical state using the knowledge and tools (AcBCDE and Head-to-toe), as well as the paths therapeutic diagnostics presented in the classroom.

The Emergency System - Territorial Urgency
The health alert system
The territorial rescue system
Systematic approach to care: ABCDE and head-to-foot assessment
Networks of time-dependent pathologies

The concept of priority in critically ill patients and emergency departments (Cosi):
In this session, starting from real cases in the field of Triage, the "global evaluation" method is developed. The objective is to convey to the student through clinical cases the ability to stratify intervention priorities and establish a path of implementation and re-evaluation based on the person's developmental risk
The context of emergency departments: ED, short intensive observation, emergency medicine.
Triage in Italy and around the world: definitions, evidence and regulations
Triage models
The rapid person assessment method: Global Assessment
The priority codes and routes, the vital signs
Fragile patients
The triage documentary system
The revaluation
Characteristics of the triage nurse

Early detection of signs and symptoms involving a potential or immediate risk to life: codes 1 and time-dependent pathologies (clinical cases)
Tools for assessing people's clinical and developmental risk: NEWS/NEWS2 (Modified Early Warning Score), APACHE II (acute physiologic assessment and chronic health evaluation), QSOFA/SOFA (sequential organ failure assessment score), SAPS II assessment scales (Simplified Acute Physiology Score II), CPS (Chest Pain Score).
CLINICAL CASE person with DYSPNEA and healthcare response
Notes on the use of ultrasound in personal care practice
CLINICAL CASE person with heart failure: care planning and response to needs

Clinical case discussion: the person with acute respiratory support (Cosi)
Starting from the data collection of a clinical case of a person with acute respiratory problems, we want to delve into the evaluation, planning and implementation of assistance and the evaluation of the results, motivating the main interventions related to the needs of people being treated with HFNC, CPAP and NIV, declining all care needs. The student will also acquire the basic knowledge to choose the type of monitoring, the frequency and the relative limits, inserting them into the overall assessment of the person and will be able to monitor the patient's evolution, detect signs of improvement/worsening and plan effective care.
Monitoring and evaluation data collection, signs and symptoms
Planning of care for the person with ventilation support through NIV, CPAP and HIGH FLOWS
CLINICAL CASE person with COPD exacerbation: care planning and response to needs
Discussion of clinical care case: the critically ill patient with neurological pathology
The student will be able to describe the techniques used for neurological evaluation and alterations in the state of consciousness. You will be able to plan nursing care for the care of the person with alterations affecting the nervous system.

Clinical monitoring and evaluation
Neurological rating scales
Intracranial pressure (ICP) and detection methods
Intracranial hypertension
Cerebral perfusion pressure (PPC).
Advance treatment instructions
The process of organ and tissue donation
