Syllabus

Course Objectives

The course is aimed at analyzing the healthcare aspects relating to the adult/elderly person who is affected by medical-internal pathologies. The course will refer to the nursing skills identified by the Tuning Educational Structures in Europe Project which, in the 2nd year of the course, include: knowledge and understanding, ability to apply knowledge and understanding, independent judgement, communication skills and learning ability. The organization of the contents will be implemented starting from the fundamental needs of the person and from healthy and protective lifestyles, coming to understand the modification of the vital and functional capacities linked to the pathologies with the greatest epidemiological evidence today, such as diabetic, hepatic, respiratory, cerebrovascular. Fundamental will be the understanding: of the methods implemented by the person, caregiver, family to recover the greatest possible vital and functional autonomy in the ability to satisfy needs
health and life maintenance treatments to be implemented, restorative treatments and diagnostic and therapeutic treatments implemented by the nurse in collaboration with the doctor and other healthcare professionals. The communicative-relational, ethical-deontological and safety aspects will be transversally implemented within the treatments.

Course Prerequisites

The required and recommended knowledge includes the contents of the 1st year disciplines: General Nursing, Theories of nursing and relational educational foundations; Clinical Nursing I and Clinical Nursing II; ethics and deontology; professional organization. They are also required basic knowledge of the disciplines of the 1st year: anatomy, physiology, and clinical pathology, microbiology and immunology.

Teaching Methods

Interactive lessons with the slide support and / or movies; exercises in plenary and / or in small groups of care situations that reproduce the main clinical pictures of the medical area, in order to develop the critical clinical-care reasoning necessary to plan nursing care. Practical laboratories

Assessment Methods

The final assessment consists of an oral test consisting of:
●a first part, which will focus on the evaluation of the theoretical knowledge detailed by the program, the passing of which will allow access to the second part;
●a second part in which the discussion of a medical area treatment situation relating to one of the pathological conditions addressed in the classroom will be addressed.
Students with DSA certified and recognized by Unipv's SAISD Center will be able to take advantage of the compensatory measures recognized to them.
Passing the CI of Clinical Medical Surgical Nursing will allow, together with the other preparatory procedures set out in the Teaching Regulations, to access the internship exam and, therefore, access the following year.

MODULE 1. Health promotion, chronic disease, patient and family education. Reception of the person at a healthcare or local facility: the treatment path of the person undergoing hospitalization for an internal medicine pathology: Urgent, ordinary, scheduled hospitalization (also with a day/day-hospital cycle), Home hospitalization. Informed consent to nursing care: forms of consent, legal foundations, responsibility. Identification of the patient at the time of reception and in all phases of care: In the inpatient unit, In the outpatient clinic, In the DH. Discharge planning and continuity of care.
MODULE 2. Pharmacotherapy: Nursing responsibility and competence in the administration of a prescribed drug. Drug management process in the clinical setting (pharmacological prescription, safe administration, replenishment and storage). Narcotics: management methods, routes of administration, side and adverse effects. Patient adherence to therapy: influencing factors, methods to ascertain it and encourage it
Routes of administration: orally; enterally: administration of drugs through nasogastric tube or PEG, oral cavity treatment in the person carrying SNG or PEG; parenteral route: intramuscular route, intravenous route, subcutaneous route, intradermal route. Indications and characteristics of the main infusion solutions. Solutions for peripheral parenteral nutrition. Preparation of the infusion route. Procedures for preparing and administering the solution to be infused. Maintenance of the infusion route with peripheral and central access. Complications during infusion therapy and preventive measures. Administration of drugs by other routes of administration (inhalation, sublingual, topical, transdermal, ocular, auricular, nasal, rectal). Calculations associated with drug therapy and patient care. Calculation of infusion rates and times. Water balance calculation. Transfusions: Purpose and purpose, phases of the transfusion process, Roles and responsibilities in requesting concentrates (informed consent), patient preparation, preparation of pre-transfusion samples, administration and infusion, monitoring and surveillance during transfusion, Transfusion errors and Transfusion reactions : nursing intervention methods, types of transfusions, purpose and methods of administration.
MODULE 3. Change in the vital need to breathe following the onset/presence of chronic respiratory, diabetic, cerebrovascular and hepatic pathology: Assessment on the person aimed at identifying the possible factors that can influence the state of oxygenation and the degree of breathing. Containment and prevention of the risk of falls related to cognitive impairment secondary to hypoxia/hypercapnia and alteration of the acid-base balance, assistance to the person undergoing non-invasive ventilation (NIV) for the correction of hypercapnia, safe administration of oxygen therapy. Interventions and care in case of: acute and chronic hypoxia (encourage to stop smoking, keep away allergens, motivate regular physical exercise, agree and implement an oxygen administration plan, educate on the constant execution of breathing exercises, agree on a nutrition plan adequate, encourage regular intake of medications, educate on the recognition of early symptoms of worsening of respiratory function), bronchial obstruction (maintain adequate hydration, carry out regular physical activity, encourage postural changes and ensure early ambulation, teach controlled coughing techniques or assisted, perform thoracic physiotherapy techniques in collaboration with the physiotherapist, motivate constant hygiene care of the oral cavity, humidify the inhaled air, motivate regular intake of prescribed medications). Monitoring for signs and symptoms of metabolic acidosis and alkalosis. Monitor for signs and symptoms of respiratory acidosis and alkalosis. Interventions and care in the management of long-term oxygen therapy (LTOT).
Education interventions on the use of non-mechanical ventilation at home. Strategies to encourage adherence of the person with COPD to the therapeutic program: global approach to pulmonary rehabilitation. NIV care management. Interventions and care in the management of patients with ascites. Change in the vital need for nutrition in a person who feeds himself through natural ways (orally) following the onset/presence of chronic respiratory, diabetic, cerebrovascular and hepatic pathologies: mechanism of action of insulin and glucose (biohumoral values reference), role of etiopathogenetic factors, unhealthy lifestyles as determinants in the onset of diabetic pathology (information and education), optimal composition of the diet in the person suffering from diabetes, according to recent scientific evidence (carbohydrates, fibre, proteins , fat, sodium, alcohol) at home and hospitalized, influence of the glycemic trend in the person undergoing enteral nutrition (risks, symptoms, monitoring), influence of the glycemic trend in the person undergoing parenteral nutrition (risks, symptoms, monitoring), positive and negative effects of nutrition on respiratory health (sodium, transfatty acids, omega-6, antioxidants, omega-3, magnesium), nutritional screening for people suffering from chronic respiratory diseases (obesity, malnutrition), motor and cognitive difficulties in eating, dysphagia, lack of appetite: strategies and personal care to restore lost or diminished ability in the event of cerebrovascular disease
restoration and maintenance of healthy lifestyles especially if present use/abuse of alcohol and unhealthy foods: concomitant risk of malnutrition, role played by nutrients in the onset of liver disease: sugars, fats, proteins, sodium (biohumoral reference indices ).

Change in the vital need to communicate in the person following the onset/presence of chronic respiratory, diabetic, cerebrovascular and hepatic pathologies: Assessment of the ability to communicate and express oneself (identification of any language alterations), Interventions and nursing care in the management of disorders of language, interventions and nursing care in the management of dyspnea which can compromise the relationship. Change in the vital need to eliminate from all excretory tracts in the person following the onset/presence of chronic respiratory, diabetic, cerebrovascular and hepatic pathologies: Assessment of the ability to control urinary and intestinal elimination, Interventions and nursing care in the management of disorders (e.g. incontinence, urinary retention, changes in stool and urine). Change in the vital need to sleep and rest in the person following the onset/presence of hepatic, cerebrovascular, diabetic and respiratory pathology: Assessment of the characteristics of the sleep-wake rhythm and the positions assumed by the patient, Interventions and nursing care in the management of the problems linked to the reference pathology. Change in the vital need to provide personal hygiene and dress/undress in the person following the onset/presence of hepatic, cerebrovascular, diabetic and respiratory pathology: Assessment of the patient's ability to satisfy self-care, Interventions and nursing care in management of problems related to the reference pathology.

MODULE 5
Laboratory 1
Blood sampling and vascular access. Venipuncture (criteria for choosing the puncture site depending on the purposes, methods, material needed, preparation and assistance to the person, consent). Blood sampling: scientific evidence for carrying out the procedure (factors and variables to consider, pre-analytical, analytical, post-analytical phase); types of blood tests and order of tube filling. Recommendations to limit errors in sampling (identification of the person, biological risk, etc.). Peripheral and central venous access: types (cvp, midline, tunneled and non-tunneled cvc, picc, port a cath, dialysis catheters and characteristics, indications and criteria for choice/positioning, management, medications, washing, blood sampling from vascular accesses; complications resulting from vascular access (pain, infection, exit site evaluation, .) and from infusion therapy (extravasation, infiltration, phlebitis). VIP SCORE (Visual Infusion Phlebitis Score). Parenteral nutrition (show nutrition bags and comment on them). no touch and sterile for accesses.
