Course Objectives

Topics and objectives: - Recognize the elements of differential clinical practice (or structural diagnosis) with minors and adults; - Assess the knowledge of minors' family members to address subjective anxieties; - Evaluate, through follow-up, the effects of clinical interventions (residential, outpatient) in the après coup;

Course Prerequisites

It is important to have a general knowledge: • of psychoanalytic theory, particularly the distinction between phenomenological diagnosis and psychic-differential diagnosis: • of the teaching of Sigmund Freud and Jacques Lacan.

Teaching Methods

Strategies and Actions: The course is delivered through in-person lectures, which illustrate, using specific clinical examples, how to define the structural diagnosis indicated by Sigmund Freud and Jacques Lacan. Reference is made to: The Presentation of the Sick, as Jacques Lacan reorganized it from the psychiatric tradition of Gaëtan Gatian de Clérambault; the practice and discussion of clinical cases obtained in applied psychoanalysis (e.g., in treatment in therapeutic communities).

Assessment Methods

WRITTEN or ORAL: Learning assessment is conducted through a written essay or an interview - chosen by the student - on the topic of the lectures/exercises.

Presentation: From medical diagnosis to psychoanalytic psychopathology. Clinical work is ethical; if it is not ethical, there is no clinical practice. The ethical significance of clinical practice is the effect of the scandal Freud brought to medical discourse by demonstrating that psychic symptoms the psychic symptom has to do with satisfaction, with enjoyment. For this reason, psychoanalysis is not guided by standards and treatment protocols. It will be possible to demonstrate how the psychoanalytic corpus, in the teachings of Freud and Lacan, possesses a knowing how to deal with the truth of the subject. Appropriate anamnesis should be understood as an ethical-clinical investigation useful for listening to the patient's words, both in the context of pure and applied psychoanalysis. In medicine, anamnesis is used for diagnosis and identifies the causes of an illness in order to address them. In psychiatry, child neuropsychiatry, and psychology, the necessary anamnesis serves to describe the phenomenology of a given "symptomatic" behavior without being able to pinpoint the underlying cause. Freud will address neurosis, but he provides an exemplary commentary on the "Schreber case" text, highlighting the traumatic issue of the subject as a speaking being, thus paving the way for psychoanalytic psychopathology. Lacan, as a psychiatrist, encountering Freud's teachings, understands that psychoanalysis is also useful for the treatment of psychosis. With him, one can learn the value of differential diagnosis by discovering the connection between symptoms and the psychic structures underlying them, so that the neurotic inhabits language while the psychotic is inhabited by it (J. Lacan, The Sem., Book III, The Psychoses, 1955-56, p. 298). The body, with its symptoms, in fact, does not only speak to hysterics. The subject structures himself through his symbolic Other and the enjoyment (the effects of the speaking body). Listening to the speaking body requires using approaches other than biomedical investigations. Furthermore, phenomenological diagnoses raise the risk of stigma. The proposed teaching approach is based on the value of singularity and "what is needed to understand the logic of the case" (Guy Briole, 2021, p. 9); it emphasizes the modalities of Clinical conversation proposed by Jacques-Alain Miller and the Clinic under Transference. The resulting anamnesis collection is part of the treatment process. The logical differences in case management between neurosis and psychosis are demonstrated and discussed.
